The Schaumburg Fire Department and Nursing Division have developed an emergency information form for Schaumburg residents. Through the use of this form, you and other family members will be able to provide public safety and health care professionals important information in an emergency situation.

When 911 emergency help is called to your home, information on the form advises the paramedics who should be called and what medications have been prescribed by your physician. Consider investing a few moments to complete the form then print, fold, and attach to your refrigerator.

Emergency information forms and magnetic holders are also available at the Village of Schaumburg Nursing Division Building at 746 E. Schaumburg Road.
